You will coordinate daily patient flow, including admissions, discharges, and transitions between levels of care, prioritizing needs to support optimal outcomes and a smooth patient experience. Leveraging your clinical expertise, you will provide triage support both in-person and through phone or electronic communication, guiding patients and families with clear, compassionate communication. In this role, you will collaborate closely with physicians, advanced practice providers, therapists, pharmacists, and support staff to ensure integrated, high-quality care in a fast-paced general medicine setting. You will maintain accurate and timely documentation in the electronic health record (EHR), uphold regulatory and organizational standards, and actively contribute to quality and performance improvement initiatives. As a clinical leader at the bedside, you will help shape and implement evidence-based protocols, champion patient safety and infection prevention practices, and escalate concerns when needed.
