CarePartners is seeking a compassionate and highly organized Registered Nurse Case Manager for our PACE Home Care program. In this role, you will coordinate and deliver comprehensive, patient-centered nursing care to older adult participants in their homes, supporting the Program of All-Inclusive Care for the Elderly (PACE). You will conduct in-home nursing assessments, develop and update individualized plans of care, and collaborate closely with an interdisciplinary team of physicians, social workers, therapists, and other providers to ensure that participants receive safe, high-quality, and cost-effective services. As an RN Case Manager, you will monitor changes in participants’ conditions, manage chronic diseases, provide medication reconciliation and teaching, arrange necessary home health services, durable medical equipment, and community resources, and advocate for participants and their families to promote independence and quality of life. You will coordinate transitions of care from hospital, skilled nursing, or rehab back to the home, ensuring continuity, clear communication, and timely follow-up. Documentation of all assessments, interventions, care plans, and outcomes in the electronic medical record is expected to be accurate, timely, and compliant with regulatory and payer requirements. Aligned with CarePartners’ culture of collaboration, compassion, and community health improvement, you will serve as a key liaison among participants, caregivers, and the CarePartners provider network, promoting strong relationships and efficient service delivery. You will work closely with internal teams to identify gaps in care, address barriers to access, and support quality improvement initiatives and population health goals. The RN Case Manager role requires strong clinical judgment, excellent communication skills, and the ability to work independently in the field while maintaining close coordination with the PACE center and partner providers. This is a full-time, field-based position with regular travel to participants’ homes within the local service area, occasional visits to partner facilities, and some flexibility to complete documentation and care coordination tasks remotely.
